29 CFR 1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: At the roofing jobsite located at 8000 Granby Ave, Building Five in Kansas City, Missouri: Employees were exposed to eye injury hazards. Employees were observed using nail guns while performing roofing work without apparent eye protection. Flying debris or fasteners from nail gun operation could strike employees in the eyes.

Employees were observed working on a roof while installing roofing materials at height without fall protection. Employees were exposed to a fall distance of approximately 22 feet from the roof eave to the ground.
