29 CFR 1926.451(c)(2)(i): Employer failed to ensure scaffold footings were level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement: a) Left side of the structure: On or about January 26, 2026, employer exposed employees to a fall hazard of approximately 13-feet in that the work platform (pump-jack scaffold) did not have mud seals under the support pole.

29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Left side of the structure: On or about January 20, 2026, the employer exposed employees to fall hazards of 13-feet to the lower level, in that fall protection was not provided for employees who were working from a pump jack scaffold used to install siding.
