29 CFR 1910.305(b)(2)(i):  All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear:  A.)    Film Ingest Area - On or about January 23rd 2026, and times prior to, a floor mounted electrical outlet had an unsecure top cover which allowed energized wires to be exposed outside the plane of the junction box.  This condition potentially exposed employees to electrical hazards.  B.)    Order Entry Area - On or about January 23rd 2026, and times prior to, flexible conduit had disconnected from a fitting connected to a floor mounted junction box, exposing energized wires that had insulative coverings.  This condition potentially exposed employees to electrical hazards.  C.)    Digital Media Area - On or about January 23rd 2026, and times prior to, a plastic junction box threaded fitting had disconnected from the junction box on the floor, exposing energized wires that had insulative coverings.  This condition potentially exposed employees to electrical hazards.
