29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about November 20, 2025, at 75 Elm Avenue, Salem, OH 44460, the employer failed to ensure employees performing cleaning on the Muratec Tourette Punch Press, model number M-2044EZ, serial number 11KX221100001, were protected from crushing hazards while performing tasks such as, but not limited to, cleaning each tool inside the machine as needed, removing the die holders as necessary, scraping out the slugs (unused metal), lubricating the index tools, greasing the track clamps, and adding oil to lubricate the tooling by utilizing lockout tagout procedures. b. On or about January 12, 2026, at 75 Elm Avenue, Salem, OH 44460, the employer failed to ensure employees performing maintenance activities on the Muratec Tourette Punch Press, model number M-2044EZ, serial number 11KX221100001 were protected from crushing hazards while performing tasks such as, but not limited to, troubleshooting the gripper clamps by utilizing lockout tagout procedures.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about November 20, 2025, at 75 Elm Avenue, Salem, OH 44460, employees were exposed to machine hazards associated with moving parts and crushing hazards while cleaning the Muratec Tourette Punch Press, model number M-2044EZ, serial number 11KX221100001. The employer employer failed to ensure the implement energy control application steps were followed as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements], including cleaning the machine with an air wand and entering into the machine to remove debris. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. As a result, employees were exposed to the unexpected energization of moving parts inside the machines. On or about January 13, 2026, at 75 Elm Avenue, Salem, OH 44460, employees were exposed to machine hazards associated with moving parts and crushing hazards while performing maintenance on the Muratec Tourette Punch Press, model number M-2044EZ, serial number 11KX221100001.
