29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. At the jobsite located at 2932 N 114th Court, Kansas City, Kansas, employees using a portable extension ladder to access the roof to perform framing activities were exposed to fall hazards of approximately 12 feet from the roof to the ground. The side rails of the portable extension ladder did not extend at least 3 feet above the landing surface of the roof.
