29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) Sawmill; Cut-Up Line: On January 28, 2026, and at times prior, the employer exposed employees to fall hazards of approximately 10 feet, 6 inches, in that the employer did not ensure that the platform around the operator's booth for the cut-up line was fully guarded on each side as employees operate the machine.

29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface:  a) Sawmill; Cut-Up Line: On January 28, 2026, and at times prior, the employer exposed employees to fall hazards of approximately 10 feet, 6 inches, in that the employer did not ensure that each section of the guardrail system bordering the catwalk on the cut-up line included a midrail as employees operate the machinery.

29 CFR 1910.29(b)(3): The employer did not ensure that guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail:  a) Sawmill; Cut-Up Line: On January 28, 2026, and at times prior, the employer exposed employees to fall hazards of approximately  10 feet, 6 inches, in that the employer did not ensure that the guardrail bordering the operator's booth for the cut-up line was maintained free of damage that could compromise its integrity and its ability to withstand downward and/or outward forces.

29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  a) Sawmill; Gang Saw: On January 28, 2026, and at times prior, the employer exposed employees to amputation hazards, in that chains and sprockets near the controls of the gang saw were unguarded as employees operated the machine to process lumber.

29 CFR  1910.305(a)(2)(x): Flexible cords and cables used for temporary wiring were not protected from damage:  a) Sawmill: On January 28, 2026, the employer exposed employees to electrical hazards, in that the yellow extension cord delivering power to a Master Industrial Products MH-135T-KFA forced air heater was not protected from damage  to its sheathing as employees used the equipment for heat during low-temperature weather conditions.
