29 CFR 1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: In the production area, employees engaged in grinding, and sharping activities were exposed to caught-in hazards in that the work rest opening were not maintained at a maximum of one-eight inch. The opening from the right side abrasive wheel to the work rest was found to be one-half inch.

29 CFR 1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: In the production area, employees engaged in grinding, and sharping activities were exposed to struck by hazards in that the adjustable tongue guard opening was not maintained at a maximum of one fourth inch. The opening from the abrasive wheel to the left tongue guard was found to be one inch. The opening from the abrasive wheel to the right tongue guard was found to be one and seven-eights inch.
