29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about December 18, 2025, on the north side of a residential worksite, an employee was not provided adequate instruction to recognize and avoid hazards associated with operating a skid steer industrial truck in a confined and restricted area in close proximity to workers.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about December 18, 2025, on the north side of a residential worksite, an employee was not in use of head protection while working inside of the dump bed of a dump truck while a skid steer was dumping debris in to the dump bed.

29 CFR 1926.602(a)(9)(ii): Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level nor did an employee signal that it is safe to do so. On or about December 18, 2025, and times prior to, on the north side of a residential worksite, an employee was operating a skid steer in reverse, in close proximity to workers, while removing debris from the jobsite into a dump truck and the skid steer was not equipped with an operable reverse signal alarm.
