29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about January 21, 2026, an employee was exposed to an amputation hazard while testing an elevator safety gripper and the employer had not ensured that the gripper had point of operation guarding to prevent the operator from accessing the danger zone if the set screws were not properly engaged.
