29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about January 29, 2026, in Building 400; employees were exposed to caught-in hazards when the employer did not develop and document lockout/tagout procedures for the control of hazardous energy during maintenance activities such as but not limited to; changing the fan on the Mazak VTC 300 vertical machining center.  b) On or about January 29, 2026, in the "Manual world" - Main Building; employees were exposed to caught-in hazards when the employer did not develop and document lockout/tagout procedures for the control of hazardous energy during maintenance activities such as but not limited to; changing bearing's on the Victor lathe.  NO ABATEMENT DOCUMENTATION REQUIRED

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a) On or about January 29, 2026, throughout the establishment; the employer did not conduct periodic inspections of the Lockout/Tagout procedures at least annually to verify that each procedure was adequate and properly followed by employees responsible for locking out machines during servicing or maintenance, including but not limited to; Mazak VTC 300, and Victor lathe. 29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about January 29, 2026, throughout the establishment; employees were exposed to hazards from rotating parts while operating , the 10ft Mazak lathe, 20ft Mazak lathe, and the Victor lathe while the chuck unguarded.   b) On or about January 29, 2026, throughout the establishment; employees were exposed to hazards from rotating parts while operating the the Mazak VTC300C mill while missing a side guard.   NO ABATEMENT DOCUMENTATION REQUIRED
