29 CFR 1910.178(m)(2):No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty. On or about February 12, 2026, at Zaclon LLC, 2981 Independence Rd., Cleveland, OH 44115, the employer failed to ensure that employees did not work beneath the elevated forks of a powered industrial forklift (Caterpillar Model P6000, Serial # AT13F01104) while loads were suspended. Employees were bending over and reaching under suspended flexible intermediate bulk containers (super sacks) filled with zinc oxide to cut open the bottoms of the containers and pull on the bags to ensure complete material discharge into a grated reactor area containing hydrochloric acid.
