29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) 461 State St. East Greenville, PA - On or about January 30, 2026, operators at the bulk bag discharge stations were exposed to crush by hazards when untying the bag outlets. Employees untie the outlet by reaching between the bulk bag and flow-flexer brace, creating a crushing hazard in the event the bag straps fail.
