29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. Production Floor - On or about February 9, 2026, at 11755 S. Austin Avenue, Alsip, IL 60803, the employer failed to develop and document a Hazardous Energy Control Program to include energy control procedures, employee training, and periodic inspections when employees were performing servicing and maintenance activities on the Gurutzpe GLX16.15.9 CNC Lathe machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Production Floor - On or around February 4, 2026, an employee sustained a right index fingertip amputation when the employee's finger came in contact with a keyway while inspecting the surface finish on a turned journal. The employer failed to provide guarding on the Poreba FL 42-480 Manual Lathe, Serial Number 7402-01-10004-00, thereby exposing employees to hazards created by rotating parts.
