29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about February 4, 2026, during the installation of framing material at a job site located at Lot 93 Golden Wood Way, Uniontown Ohio 44685; the employer did not ensure employees working on the roof of a single-family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:   On or about February 4, 2026, during the installation of framing material at a job site located at Lot 93 Golden Wood Way, Uniontown Ohio 44685; the employer did not ensure employees working at an elevated height standing on material resting on the forks of a CAT TL1055C Construction Telehandler (All Terrain Forklift) were protected from om fall hazards.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about February 4, 2026, during the installation of framing material at a job site located at Lot 93 Golden Wood Way, Uniontown Ohio 44685; the employer did not ensure employees working on a roof of a single-family home were protected from a fall of greater than 17 feet by utilizing a fall protection system.
