29 CFR  1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  On or about February 4, 2026, employees utilized equipment with flexible cords and cables  that were not protected from accidental damage in the following instances:  a) In the administrative office area leading to the warehouse area at building #5, a yellow 120V (12 gauge / 100 foot) extension cord was found pinched between the door going into the warehouse.  b) In the warehouse area at building #5 dock door 1038, an orange extension cord was pinched by the dock door leading to the exterior of the building.
