29 CFR  1910.147(c)(1):  The employer did not establish an program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Facility - The employer did not have an energy control program (lockout tagout) for equipment such as but not limited to the lathes, milling machines, robo drill and other equipment.  Additionally, equipment was not placed in a state where the unexpected energization was not implemented and secured.  Employees were not trained in the requirements of the standard to prevent electrical and crushing injuries.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Facility - Employees were exposed to rotating parts and ingoing nips on several lathes which were not adequately guarded.

Safety of equipment shall be determined using the following considerations:  Facility - The circuit breaker panel located next to the SPA 10 lathe was missing the cover and had unguarded openings, exposing employees to live electrical parts.
