29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a)7 Sons, LLC at 640 Arizona Avenue Northwest, Huron, SD 57350: On or about and at times prior to January 26, 2026, the employer did not ensure employees were adequately instructed to recognize and avoid hazards associated with the installation, maintenance, testing, and removal of equipment on pressurized piping systems. This condition exposed employees to explosion and struck-by hazards.
