29 CFR  1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  At the aluminum area, employees engaged in grindings activities are exposed to noise related health hazards when exposure levels were above the OSHA Action Level (AL), The employer did not conduct a an audiogram for each employees that is above the AL.  Note: The 8-hour TWA sound level of 85 dBA that triggers the requirement for a hearing conservation program (HCP) can be adjusted for work shifts less than or greater than 8 hours. When making the adjustment, the following formula is used: AL = 90 + 16.61 log [50/10 (hours)].  Affected employees were exposed to continuous noise levels over a 10-hour work shift.  Therefore, the TWA sound level that triggers the requirement for a HCP is adjusted or reduced for the affected employees to 83.4 dBA.  1) On or about February 19, 2026, an employee was exposed to a 10-hour time-weighted average sound level of 89 dBA. The sampling was performed for 568 minutes. 2) On or about February 19, 2026, an employee was exposed to a 10-hour time-weighted average sound level of 89.6 dBA. The sampling was performed for 575 minutes. 3) On or about February 19, 2026, an employee was exposed to a 10-hour time-weighted average sound level of 91.7 dBA. The sampling was performed for 527 minutes.

29 CFR  1910.1000(a): 29 CFR 1910.1000(a):  The employer shall ensure that no employee is exposed to any substance at a level in excess of the exposure limit given for that substance in Table Z-1 of this standard.  On or about February 19, 2026, at the hand welding frame weldment area, an employee was exposed to copper fumes related health hazards when exposure levels were above the eight-hour time-weighted average (8-hour TWA) permissible exposure limit (PEL) of 0.1 milligrams per cubic meter of air (mg/m�). The employee's 8-hour TWA exposure was 0.1889 mg/m�.

29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  At the research and development area, employee(s) engaged in grinding activities are exposed to struck-by and caught in hazards in that the tongue guard was adjusted at 5/8 of an inch from the grinding wheel.
