29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: On or about February 6, 2026, employees were exposed to electric shock hazards and the employer did not ensure energized electrical outlets were equipped with faceplates: a. A (1 of 2) 110V outlet was being used to power temporary lighting near the 100-K location on the first floor and was not equipped with a faceplate. b. A (2 of 2) 110V outlet was being used to power temporary lighting near the 100-K location on the first floor and was not equipped with a faceplate. c. A 110V outlet was being used to power temporary lighting near the 100-J location on the first floor and was not equipped with a faceplate. d. A 110V outlet was being used to power temporary lighting near the 100-L location on the first floor and was not equipped with a faceplate. e.
