29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: On February 9, 2026, employees at Cowles Industrial Tool were exposed to respiratory hazards associated with inhaling vapors while working with Starfire Talex EP Gear Oils, Starfire AW 32/46/68, Delta Cool 120, and Nanotech 6800. The employer failed to evaluate the hazards in the workplace to determine the employees exposure to the chemicals through personal air monitoring.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about February 9, 2026, employees working in the flat area at Cowles Tool were exposed to fire hazards. The portable fire extinguisher in that area was not readily accessible as required. It was obstructed by a 55-gallon drum of AW 68, an out of service Bridgeport machine, and a shelving unit, preventing employees from accessing it without potential injury.
