29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about February 9, 2026, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about February 9, 2026, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately eleven (11) feet, above the ground, thereby exposing employees to fall hazards.  b.  On or about February 9, 2026, at the above addressed jobsite, an employee engaged in roofing activities was not protected from fall hazards while working at an elevation of approximately twelve (12) feet above the ground, thereby exposing the employee to fall hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about February 9, 2026, at the above addressed jobsite,  employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
