29 CFR 1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or before February 20, 2026, De Leon Unlimited, Inc. dba Global Multi-Family/Construction/Renovation as the controlling employer, did not ensure subcontractor employees were provided eye protection when stripping roofing debris and using pneumatic nail guns during roofing operations. The condition exposed employees to the hazard of foreign debris in their eyes and struck by projectiles from the nail guns.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or before February 10, 2026 De Leon Unlimited, Inc. dba Global Multi-Family/Construction/Renovation as the controlling employer did not ensure that subcontractor employees were protected from injury, in that employees removing and installing shingles on a three-story residential apartment complex, with a pitched roof, were not utilizing a fall protection system. This condition exposed the employees to falls greater than 30 feet.
