29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about February 9, 2026, at the above addressed jobsite, employees engaged in residential roofing operations were exposed to eye or face hazards from flying particles while operating a pneumatic nail gun without eye protection.  Silver Line Exteriors & Remodeling Inc. was previously cited for a violation of this occupational safety and health standard 1926.102(a)(1), which was contained in OSHA inspection number 1743664, citation number 1, item number 1 and was affirmed as a final order on August 20, 2024, with respect to a workplace located at 5619 Hillcrest Road in Downers Grove, Illinois, 60515.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about February 9, 2026, at the above addressed jobsite, portable ladders used to access the roof at an elevation of nine (9) feet did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls. Employees were exposed to fall hazards in excess of nine (9) feet while performing roof installation activities without using fall protection.
