29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Site- On or about December 4, 2026 an employee fell approximately 21 feet while working from the roof without fall protection.

29 CFR  1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  Site- On or about December 4, 2025 employees working from the roof were not protected from bodily injury where personal fall arrest equipment was used, which included connecting lanyards to D rings located at the employee's waist.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Site- Employees using aluminum extension ladders to access roof were not protected from fall hazards where the ladders did not extend three feet above the landing surfaces.
