29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about February 12, 2026, the employer failed to ensure that employees were utilizing fall protection while performing framing operations from the basket of a JLG Lift (Model:800S). Employees were exposed to catapult and subsequent fall hazards of approximately 10 feet while performing work without the use of fall protection.
