(a) On the roof of the building - On or about February 12, 2026, employees were exposed to struck-by hazards, in that the employer did not ensure employees wore eye protective equipment while using pneumatic nail guns.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On the second floor deck - On or about February 12, 2026, the employer exposed employees to a fall hazard, in that employees did not use fall protection while installing floor joists on a residential home with a 9'10" fall to ground distance.
