29 CFR 1904.29(b)(3): The employer did not record a case on their [OSHA Form (300 and/or 301] or equivalent within seven calendar days of receiving information that a recordable injury or illness had occurred: (a) Buchman Lumber Company, LLC - N9421 North Road, Springbrook, WI 54875; On or about February 3, 2026, an employee sustained a work-related left arm fracture that met the general recording criteria. The employer received information that a recordable injury had occurred on February 3, 2026, but did not enter the case on the OSHA Form 300 until nine calendar days later, on February 12, 2026.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Throughout the sawmill; On or about February 12, 2026, and dates prior, written energy control (i.e. lockout/tagout) procedures were not evaluated within the previous year for equipment including, but not limited to, the head saw, resaw, edger, trim saw, conveyance equipment, etc.
