29 CFR 1926.20(b) Accident Prevention Responsibilities. (2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about February 2, 2026, the employer failed to ensure that frequent and regular inspections of the job site were conducted. Manholes with unsecured plywood coverings were not identified and corrected. As a result, five employees performing dewatering work on on or around the unsecured manhole were exposed to fall hazards of 8 feet.

(3): All covers shall be secured when installed so as to prevent accidental displacement by the wind, equipment, or employees. a) On or about February 2, 2026, the employer failed to ensure that plywood, used as manhole covers, was secured to prevent accidental displacement. As a result, employees were exposed to fall hazard of approximately 8 feet into the manhole.
