29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: a) Fabrication Area - On February 17, 2026, the employer permitted an employee to wear a 3M 8210 Plus N95 filtering facepiece respirator with facial hair that interfered with the seal between the face and the respirator's facepiece. b) Fabrication Area - On February 17, 2026, the employer permitted an employee to wear a 3M 6200 series tight-fitting half-face respirator, equipped with a 3M 2097 particulate filter (P100), with facial hair that interfered with the seal between the face and the respirator's facepiece. Employees are required to utilize respiratory protection during the fabrication of stone countertops. Florenza Marble & Granite Corporation was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.134(g)(1)(i)(A), which was contained in OSHA inspection number 1730297, citation number 1, item number 4b and was affirmed as a final order on June 6, 2025, with respect to a workplace located at 1900 North Austin Avenue Suite 70, Chicago, IL 60639 . In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.1053(d)(6)(i): Within 15 working days after completing an exposure assessment in accordance with paragraph (d) of this section, the employer did not individually notify each affected employee in writing of the results of that assessment or post the results in an appropriate location accessible to all affected employees: a) Fabrication Area - On or before February 9, 2026, the employer did not notify each affected employee working in the fabrication area in writing or by posting of the results of the air monitoring that were received via email from the Certified Industrial Hygienist (CIH) on January 16, 2026. Florenza Marble & Granite Corporation was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.1053(d)(6)(i), which was contained in OSHA inspection number 1730297, citation number 1, item number 7 and was affirmed as a final order on June 6, 2025, with respect to a workplace located at 1900 North Austin Avenue Suite 70, Chicago, IL 60639 .
