29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   (a) Under-construction ladder-frame pole building; On or about February 17, 2026, and dates prior, a means of fall protection was not provided during framing activities for employees who worked from wall top plates, roof trusses, girts, and purlins at approximate heights ranging between 16-feet to 25-feet above ground level.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  (a) Under-construction ladder-frame pole building; On or about February 17, 2026, and dates prior, employees were not provided with a safe means of access to elevated work areas and were required to climb horizontal wall girts and/or bracing members spaced approximately 24 to 30 inches on-center, with heights ranging between 16 feet (girts) to 22 feet (bracing) above ground level.
