(a) On or about November 3, 2025, employees performed work on existing stairway lighting but the circuit was not kept deenergized.  (b) On or about January 20, 2026, employees conducted electrical work on a light switch on the roof of Penthouse 3 and 4 but the circuit was not kept deenergized.
