29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) In the warehouse on the site located at 12685 Tower Road, Bonita Springs, FL: On or about 02/18/2026, the employer exposed an employee to crushing hazards, in that, employees were operating a forklift with a basket attachment that without written approval from the manufacturer. There was also no load capacity chart for the alteration.
