29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees engaged in maintenance activities at the following pieces of equipment throughout the facility of the Front and Back Machine Shop areas were exposed to electrical, struck-by and caught-between hazards in that machine specific energy control procedures had not been developed and implemented. Multiple equipment including, but not limited to, Cincinnati 45 CNC Bridge Mill, Mori Seiki MV-65 CNC Mill, Cincinnati Milacron 30VX CNC Mill, Smart SM-312 CNC Mill, Cincinnati 30V CNC Mill, Mori Seiki NV-4000 CNC Mill, OKK 5-24 VP-600 CNC Mill, OKK VB-53 CNC Mill, Doosan DVF-5000 CNC Mill, Doosan DNM-5700CNC Mill, Mazak Vari-Axis 730-5XII CNC Mill, Haas VF5 TR CNC Mill, Smart SM-565 CNC Mill, Okuma Millac 800VH CNC Mill, which had multiple energy sources including pneumatic, hydraulic, electric, gravity.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees engaged in machining activities at the following pieces of equipment throughout the facility were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized: a) Bridgeport drill press in the Assembly Room, the rotating drill chuck was not guarded. b) Alliant drill press in the Manual Machine area, the rotating drill chuck was not guarded. c) Acer Bridgeport drill press in the Manual Machine area, the rotating drill chuck was not guarded.
