29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of 1,2-Dichloroethylene listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 200 PPM. (a) On February 18th, and at times prior, at 1251 South 48th Street Grand Forks, ND, in the mattress assembly area, a mattress assembler was exposed to 1,2-Dichloroethylene at an 8-hour TWA of 310 ppm of air, approximately 1.5 times the PEL of 200 ppm while spraying Tensorgrip P390 glue.

29 CFR 1910.178(i)(1): Concentration levels of carbon monoxide gas created by powered industrial truck operations exceeded the levels specified in 1910.1000: (a) On February 18th, and at times prior, at 1251 South 48th Street Grand Forks, ND, employees working in the mattress shop, loading and unloading materials, and assembling mattresses were potentially overexposed to high levels of carbon monoxide where the Toyota Forklift: Model: 8FGUI8, S/N: 119436 had carbon monoxide emissions of approximately 249 ppm.
