29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about February 19, 2026, an employee was working from a personnel basket that was hoisted 15 feet above the ground below and the employee was not protected from falling by fall protection established in paragraphs 1926.451(g)(1)(i)-(vii).

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about February 19, 2026, two employees engaged in installing roof sheathing on a residential building were exposed to fall hazards as much as 17' above the next lower level were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).

29 CFR  1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  On or about February 19, 2026, an employee was elevated approximately 15 feet above the ground in a personnel basket that was placed on the forks of a Cat TL642 telehandler and the platform was not secured to the lifting carriage and/or forks.
