29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Sawmill, chipper area; On or about February 18, 2026, and dates prior, an energy control program with procedures, employee training, and periodic inspections was not established and implemented before employees performed covered activities, including but not limited to unjamming and sharpening knives on the Precision Chipper (Serial No. C.1111), which were performed without locking out the equipment. (b) Sawmill, head saw area; On or about February 18, 2026, and dates prior, an energy control program with procedures, employee training, and periodic inspections was not established and implemented before employees performed covered activities, including but not limited to sharpening teeth on the Head Saw, which were performed without locking out the equipment. (c) Sawmill, resaw area; On or about February 18, 2026, and dates prior, an energy control program with procedures, employee training, and periodic inspections was not established and implemented before employees performed covered activities, including but not limited to unjamming and changing blades on the Wood-Mizer Products Multi-Head Resaw (Serial No. MH6D12 1205087A3.01), which were performed without locking out the equipment.
