29 CFR 1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: Location: 39 Pine Grove St, Needham, MA On February 20, 2026, workers were exposed to fall hazards of up to 20 feet to the ground while using ladder jack scaffolding to conduct roofing activities where fall protection was not used.

29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. Location: 39 Pine Grove St, Needham. Rear of house On February 20, 2026, Employees accessed a ladder jack scaffold by crawling out an attic window, across the roof, and dropping down onto the scaffold. There were no ladders or other dedicated means for accessing the scaffold.
