29 CFR 1926.452(m)(1): Crawling boards did not extend from the roof peak to the eaves when used in connection with roof construction, repair, or maintenance: On February 18, 2026, at 9 McCall Terrace, St Louis, MO, 63105, and at times prior, employees were exposed to fall hazards from crawling boards which did not extend from the roof peak to the eaves during residential roof repair. Employees were using pieces of extension ladders as crawling boards and chicken ladders to gain access to a platform near the peak of a roof, exposing them to falls when transitioning work surfaces at the eave and at the peak.

29 CFR 1926.452(m)(2): Crawling boards were not secured to the roof by ridge hooks or by means that meet equivalent criteria: On February 18, 2026, at 9 McCall Terrace, St Louis, MO, 63105, and at times prior, employees were exposed to fall hazards from crawling boards that were not secured to the roof by ridge hooks or by an equivalent means. An extension ladder being used as a crawling board was affixed to the roof utilizing a piece of metal wrapped around the top rung of the ladder and affixed to the roof using five to six nails through the metal.

Employees were working from an elevated surface on a 10 to 12 pitch, slated shingle roof approximately 30 feet at the peak and 12 feet at the eave.

On February 18, 2026, at 9 McCall Terrace, St Louis, MO, 63105, and at times prior, employees were exposed to fall hazards from ladders in use for purposed for which they were not designed. Extension ladders were used as crawling boards and chicken ladders on the roof of a residential structure. A portion of an extension ladder was used as a walkboard on a residential roof with a 10 to 12 pitch and slate shingles. A five rung sawed off ladder was also used as a chicken ladder.
