On or about February 19, 2026, at the above addressed jobsite, employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

On or about February 19, 2026, at the above addressed jobsite, employees engaged in roofing operations were not protected from fall hazards while working at heights exceeding twenty-five (25) feet, above the ground, thereby exposing employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a. On or about February 19, 2026, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall hazards, thereby exposing the employees to the hazards associated with falls.
