29 CFR 1910.147(c)(8): Lockout or tagout was not performed by the authorized employee(s) who were performing the servicing or maintenance. On or about February 13, 2026, while performing service work on a KM Line re-saw machine at 1901 Progress Drive, Farmington, Missouri, an employee was exposed to caught-in or -between hazards from moving machine parts. The employer did not ensure employees used Lockout or Tagout (LOTO) procedures to control hazardous energy and isolate the machine before performing service or maintenance.
