The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  On or about February 18, 2026, the employer did not ensure that employees connected their harnesses to fall protection anchorage points correctly.  Employees performing work at heights in approximately 12 feet had their lanyards attached to the front side of their harnesses, exposing them to serious injury in the event of a fall.  No abatement certification or documentation is required for this item.
