29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. McKinley Drive, New Britain, CT 06053: On or about September 02, 2026, Employer did initiate and maintain a safety program facilitating frequent & regular inspections of the worksite, materials, and equipment by a competent person resulting in an Employee injury. A) 85 McKinley Drive, New Britain, CT 06053: Employees were exposed to cave-in hazards, unauthorized equipment operation, and other safety hazards, while conducting Pile Driving operations within an excavation which did not have adequate protective systems in-place. Employer did not ensure Sub-Contractor had been following the project plans and guidelines provide by the Employer and approved by the City. B) 197 Governor Street, New Britain, CT 06053: Employees were exposed to cave-in hazards, unauthorized equipment operation, and other safety hazards, while conducting Pile Driving operations within an excavation which did not have adequate protective systems in-place. Employer did not ensure Sub-Contractor had been following the project plans and guidelines provided by the Employer and approved by the City.

McKinley Drive, New Britain, CT 06053: On or about September 02, 2026, Employer did not properly slope, sheet-pile, or brace the excavations for Pile Driving operations. A) 85 McKinley Drive, New Britain, CT 06053: Employees were exposed to cave-in hazards, unauthorized equipment operation, and other safety hazards, while conducting Pile Driving operations within an excavation which did not have Employees protected by the proper sloe, sheet-pile, or brace.
