OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards: On or about February 13, 2025, employees were exposed to struck-by and crushing hazards created by the unintended movement of heavy equipment. An equipment operator exited a Terex 760B loader backhoe without shutting off the engine, engaging the parking brake, and removing the starter switch key. The operator attempted to operate the loader bucket controls from outside the equipment cab when the machine subsequently rolled down a 7?degree slope, striking the operator and dragging the individual approximately 25 yards. The operator sustained multiple fractures and internal injuries.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Glen Echo Country Club failed to report within 24-hours a work-related incident that resulted in an in-patient hospitalization. An employee sustained bodily injuries and was hospitalized for treatment after being run over by backhoe on 02/13/2026 which was reported on 02/18/2026.
