29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer: a) Job site, 4612 Lytle Street, Hazelwood, PA: On or about February 20, 2026 during framing work, the employer did not ensure a system of hazard identification and correction was implemented at the site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection when necessary and incorrectly using a retractable device in that the user could free fall more than 6 feet and would also be exposed to a swing hazard.

29 CFR 1926.502(d)(15)(ii): Fall protection anchorage points were not installed under the supervision of a qualified person: a) Job site, 4612 Lytle Street, Hazelwood, PA: On or about February 20, 2026 during framing work, the employer did not ensure fall protection anchor points were installed under the supervision of a qualified person in that a retractable device was anchored at the very edge of the building, where an employee worked along the same edge and was able to free fall more than 6 feet and was exposed to swing hazards.

29 CFR 1926.502(d)(16)(iii): Fall arrest systems were not rigged such that an employee would not be able to fall more than 6 feet (1.8 m): a) Job site, 4612 Lytle Street, Hazelwood, PA: On or about February 20, 2026 during framing work, the employer did not ensure fall protections systems were appropriately set up and used in that an employee used a fall arrest system that was installed along the edge of the exterior top plate, then he walked a distance of at least 10 feet along the same edge of the exterior top plate, exposing him to a free fall of over 6 feet.
