29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 02/27/2026 in the production area;  where employees perform covered servicing and/or maintenance operations such as but not limited to changing saw blades on the chop saws without positioning the energy isolating means in the "off" position and locking the energy isolating means for the equipment in the "off" position. A thorough review would have identified servicing/maintenance operations where lockout was not being utilized.  ABATEMENT CERTIFICATION REQUIRED
