29 CFR 1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a) New construction located on Lot #156 Baymont Drive, Braselton, GA 30517, Ground level: On or about 2/23/2026, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that protective eyewear was not provided to employees operating these tools at 130 PSI while engaged in siding activities.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    a) New construction located on Lot #156 Baymont Drive, Braselton, GA 30517,Ground level: On or about 2/23/2026, the employer exposed employees to 120V electrical shock hazards while operating electrical tools including but not limited to circular saws, in that the ground pins to the 3-way outlet were missing and the extension cord had multiple cuts throughout the outer protective jacket while engaged in siding activities.

29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were modified in order to intermix them and a competent person did not determine that the resulting scaffold was structurally sound:  a) New construction located on Lot #156 Baymont Drive, Braselton, GA 30517,Scaffold: On or about 2/23/2026, the employer exposed employees to collapse/fall hazard of approximately 15 feet 10 inches to the lower ground level. Employees performing siding installation were working from a pump jack scaffold system that had been improperly extended. Wooden planks that were not scaffold?grade, were not part of the manufacturer?approved platform system, and had not been tested or rated for equivalent load?bearing capacity were added to extend the platform length, creating an unsafe condition and increasing the risk of a fall to the lower ground level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) New construction located on Lot #156 Baymont Drive, Braselton, GA 30517, Scaffold: On or about 2/23/2026, the employer exposed employees to fall hazards of approximately 15-feet, 10-inch to the lower ground level, in that a fall protection system was not provided to employees engaged in siding installation activities working from a pump jack scaffold system.  Daniel Alberto was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1820151, citation number 1, item number 1 and was affirmed as a final order on 6/17/2026, with respect to a workplace located at 2355 Windsor Bluffs Drive Cumming, GA 30041.
