29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) New commercial construction located at 4570 Martin Road, Cumming, GA 30041: On or about 02/24/2026, employees engaged in glass window installation activities using a JLG aerial lift were exposed to ejection/fall hazards of approximately 7 feet to the lower level, in that required fall protection/fall arrest was not used while positioned inside the lift basket.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. a) New commercial construction site located at 4570 Martin Road, Cumming, GA 30041: On or about 02/24/2026, employees performing glass window installation were exposed to fall hazards. Workers were using a JLG aerial lift to conduct window installation activities at heights exceeding 7 feet above the lower level. The employer had not ensured that these employees received the required training to recognize and avoid fall hazards associated with aerial lift use.
