29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): a)?Production?Department -?On or?about February 13, 2026,?employees were?exposed to?machine hazards?associated with?moving parts?while removing a jam from Machine #62 Potdevin Bagger Machine. An employee received a laceration to the hand while attempting to remove jam from the machine. ?The employer?failed to ensure that employees were removed from the machine or equipment area while energy isolating devices were temporarily bypassed during the testing or troubleshooting of the equipment.
