On or about February 24, 2026, an employee used a double chuck lathe which did not have a chuck guard. b. On or about February 24, 2026, an employee used a double chuck lathe which did not have a rear chuck guard. c. On or about February 24, 2026, an employee operated a band saw that was not guarded to protect the operator from the unused portion of the saw blade. d. On or about February 24, 2026, an employee used a DBRR machine which did not have a chuck guard.
