29 CFR  1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) At the shop/warehouse located at 12350 Belcher S Rd, Largo, FL 33773:  a cutter helper was exposed to continuous noise levels at 114.6 % of the allowable 8-hour TWA-Action Level (85 dBA).  The equivalent dBA of 114.6 % is 91 dBA.  The sampling was performed for 355-minutes during one shift on March 3, 2026.   b) At the shop/warehouse located at 12350 Belcher S Rd, Largo, FL 33773:  a machine operator was exposed to continuous noise levels at 67.8 % of the allowable 8-hour TWA-Action Level (85 dBA).  The equivalent dBA of 67.8 % is 87.2 dBA.  The sampling was performed for 344-minutes during one shift on March 3, 2026.    c) At the shop/warehouse located at 12350 Belcher S Rd, Largo, FL 33773:  a helper was exposed to continuous noise levels at 150.9% of the allowable 8-hour TWA-Action Level (85 dBA).  The equivalent dBA of 150.9 % is 93 dBA.  The sampling was performed for 353-minutes during one shift on March 3, 2026.    d) At the shop/warehouse located at 12350 Belcher S Rd, Largo, FL 33773:  employees were exposed to continuous noise levels at 148.9% of the allowable 8-hour TWA-Action Level (85 dBA).  The equivalent dBA of 148.9 % is 92.9 dBA.  The sampling was performed for 447-minutes during one shift on March 3, 2026.
